Coroner Emphasizes Missed Chances as Infant Passes Away After Birth Complications

Heartbreaking News: Baby Coolio’s Tragic Passing

A recent inquest revealed “missed opportunities” in care for Kimberley Phillips leading to the tragic loss of her baby, Coolio Carl Justin John Morgan, at Princess of Wales Hospital, Bridgend. Coolio died from hypoxic-ischaemic encephalopathy, a condition caused by oxygen deprivation during birth.

Kimberley, 44, had been induced due to her age at 39 weeks. Despite her pleas of severe pain and feeling unwell, complications arose, leading to an emergency vacuum-assisted delivery on a night when the hospital was exceptionally busy.

Mum Kimberley experienced severe blood loss and went through a rough time in theatre, bringing her dangerously close to losing her own life. Coolio was transferred to Singleton Hospital, facing continuous seizures due to a brain injury. Kimberley later tested positive for COVID-19.

Coroner Rachel Knight identified missed opportunities to prioritise Kimberley’s care. Although COVID-19 and recent health board changes were partly blamed, swift actions by staff when complications arose were noted. Sadly, Coolio passed on 5th May, after his breathing tube was removed with parents’ consent.

Richard Hughes of Cwm Taf Morgannwg University Health Board extended apologies, vowing to learn from such tragedies. He stressed a commitment to high standards of care.
