An inquest revealed that Mr. Bassett had previous suicide warnings due to a recent attempt to burn himself. But when prison staff found Mr. Bassett “laughing and joking,” they neglected vital procedures that could have provided crucial support.
A damning report from prisons ombudsman Sue McAllister shows how the failure to act on documented risks was a critical misstep. The jury agreed, highlighting that this oversight likely contributed to his untimely death.
