Leading up to his passing, Trevor showed erratic behaviour, prompting his family to contact emergency services multiple times. Trevor even opened up about deliberately crashing his bike and burning what seemed to be a suicide note. Despite these signals, an effective intervention never fully materialised. Trevor was taken to hospital once, but left before receiving a mental health assessment.

A mental health duty officer met with Trevor but lacked crucial background information. Unfortunately, this meant Trevor’s real risk level was underestimated. This heart-wrenching outcome leaves Trevor’s family urging for better protocols and information sharing between mental health services.

His family, supported by legal experts, underscore this case as a poignant reminder for the necessity of thorough mental health assessments and efficient service communication. They honour Trevor’s memory, advocating for systemic changes to ensure this never happens again. Their message to Trevor: “We love and miss you beyond words. Rest in peace, until we meet again.”
