Tragic loss from a preventable tragedy: Tali Cestaro, a vibrant 18-year-old with a love for musicals and cooking, tragically passed away in November 2023 after ingesting a foreign object while in a mental health facility. Despite her known history of impulsive behaviour, the hospital failed to provide timely specialist care after suspecting a partial tear in her stomach. An inquest revealed shocking gaps in communication and delays in crucial imaging, leading to her untimely death from sepsis. Tali’s family, while heartbroken, are grateful that her organ donations have saved three lives, and they hope lessons learned will prevent future tragedies.

The inquest highlighted significant failings from Coventry and Warwickshire health services, underscoring the need for improved safety planning and communication between mental health and acute care services. Both NHS Trusts involved have apologised and promised changes, but continue to face criticism for missed opportunities to save Tali’s life.

Tali’s legacy lives on, not just through her organ donations, but in the hope and determination of her family and advocates to ensure no other family endures such a loss. Remembering her as a “funny, passionate whirlwind of a girl,” her family hopes that serious improvements in patient care and accountability will prevent future heartbreaks. ❤️
