An investigation revealed that the company had neglected to maintain its equipment adequately. Shockingly, this wasn’t the first incident with failing doors at the site. Despite previous warnings, the company hadn’t set up a routine for inspections or preventative maintenance. Tony, who was not properly trained, was repeatedly allowed to perform these high-risk repairs.

The Health and Safety Executive pointed out that powered overhead doors, classified as work equipment, must be kept in a safe condition and regularly inspected. Sadly, these precautions weren’t in place, and Tony’s death could have been prevented.

Tony’s wife, Ewelina, shared the emotional toll of losing her husband. “It is still very raw,” she said. “Every day when I wake up, it feels like a cloud hanging over me.”

The case serves as a stark reminder of the importance of workplace safety and regular equipment checks. This devastating incident should urge all businesses to review their safety protocols and ensure their workers’ well-being is a priority. Our thoughts go out to Tony’s family and loved ones.
