An unconscious patient fell off the operating table during surgery at a Welsh hospital in a shocking incident that’s raising serious safety concerns. The mishap happened at the University Hospital of Wales in Cardiff, where a patient undergoing gallbladder surgery slipped off the table due to not being properly strapped in, sources say.

Thankfully, the patient wasn’t injured despite the drop, which was about four feet, ensuring there was no full impact with the floor. A full review has been launched by the hospital, but insiders claim it’s part of a worrying trend of safety lapses. Earlier this year, a surgical swab was forgotten in a patient’s throat for over 24 hours after another operation at the same hospital.
Concerns are growing over the hospital’s operating theatres, which were previously criticised for tolerating illegal activities and lacking a transparent safety culture. Staff speak of an atmosphere of fear, with new employees not receiving sufficient training and seasoned staff leaving due to a ‘toxic culture’.

While the health board denies this incident is a “never event” – a term for major, preventable errors – many are questioning the safety protocols that allowed such an accident to happen.
Critics argue that ongoing issues like these – including previous catastrophic mistakes leading to patient deaths – are symptoms of deeper systemic problems within the health board. These latest revelations add to the increasing pressure on the health board to front up and resolve persistent safety and management failures.
