As revealed in Pontypridd’s coroner’s court, Rafal, battling type 1 diabetes, administered insulin without prior blood sugar testing. This critical error led to a severe hypoglycaemic episode on January 19, ultimately leading to his untimely demise on February 7 after intensive care efforts at Prince Charles Hospital.

Coroner Rachel Knight concluded the tragedy as death by misadventure, highlighting the crucial need to monitor blood sugar before insulin use. The post-mortem confirmed that hypoglycaemia had deprived Rafal’s brain of oxygen, causing fatal damage.

Inquests, while distressing, serve a vital role by shedding light on such preventable incidents. They help raise awareness, potentially saving lives and preventing future tragedies.

Let Rafal’s story be a cautionary tale: always check your levels and stay safe. ❤️
